Tunnel Syndrome
The median nerve and hand tendons must pass through a narrowed area in the wrist to get to the hand. Injury, swelling or anything that takes up extra space or reshapes this tunnel can cause pain or weakness. Inflammation of the tendons because of repetitive use of the hand and wrist may also lead to Carpal Tunnel Syndrome.
While some people may experience a causative injury or strain, common underlying culprits include muscle overuse and poor scapular-shoulder mechanics. This problem may appear when someone repetitively performs a specific activity, thereby overusing their shoulder-elbow-arm muscles. Symptoms can include pain, tingling or numbness in the thumb, index, middle and half of the ring finger. These symptoms frequently occur at night or with activities such as driving or typing. When pressure on the nerve is relieved, symptoms rapidly disappear. If symptoms persist, the ability to pinch may be lost, and if ignored too long, the continued pressure on the nerve could cause irreversible damage.
Effective treatment may begin with rest and anti-inflammatory drugs, followed by physical therapy treatment to the neck, shoulder and arm. Therapy includes evaluation of sensation, strength, posture and shoulder-arm mechanics, with treatments including ice, massage, ultrasound, proper biomechanics and exercise. Occasionally (in severe cases) surgery may be required to relieve nerve pressure.
